You should read carefully all the information that was sent to you previously regarding state appeals. After this, find out what type of information they require as it depends on state to state.
You should always include on your letter the date, your own address and any other person relevant. If you wish you should address the subject matter of the letter. If you also have a specific contact person add that person's details to the address.
You need to add the relevant personal details including who you are and your case number. If you have a social security number too add that as well.
Start off by stating your intentions and why exactly you are intending to appeal and the subject matter surrounding it.
Now you should move onto the main body of your letter and state clearly and concisely why you feel you have the right to unemployment benefits. You cannot be too aggressive but you should state confidently the reasons why you are entitled.
You may have been given a reason previously but you should focus on your aims and develop a good solid argument as to why you feel your last appeal was rejected.
It is always a good idea to include witnesses in your statement and if you have any documents to support your character then this would be extremely advantageous. Please mention their details and the best way to get in touch with them.
You should finish off with your final plea and explain if you are enclosing anything to the document, like witness statements, copies of letters or correspondence which may support your case. After you sign it off then please write below all the documents you are enclosing in bold and even underlined so it is clear.

I was traveling every 2 weeks from California to Kentucky.  I have chosen to quit my job to be closer to home in Kentucky. I have proof that I have continuously been seeking employment, along with several interviews since I have arrived back in Kentucky.  Do I have a case to appeal the decision for unemployment.

Do I have a case for appeal?

Here is the situation.

My employer terminated me by phone the day I was to report for work, after two years at the same job. He cited as the reason for termination, a few events which could not possibly have taken place on my shift and said that one of our clients had complained about me. Would it be possible to subpoena this client to appear at our appeal hearing, and how should I word my letter?

Do you know what the so called "events" where? Did you have prior signed discipline in your file and did you sign anything regarding the events? If your employer has no proof that your caused the seperation then you should get benefits. Of course the devil is in the details, so without knowing your history I can't say 100%.

No signed *anything*. I already submitted the appeal request letter stating exactly why the events they are claiming couldn't have happened. I could tell that she was lying over the phone, because of the pauses and going "um" and taking several seconds to come up with her story before she told it to me, but the question will be if I'm screwed because it may be my word against hers. She only told the EDD that I "did not perform my duties". A creditor who called work said I'd been laid off...

How do you start the letter off and do I need to provide the info when I file the appeal or wait until the hearing is scheduled?

Online on your account under determination at the bottom. You click on view and there will be a link to appeal or on the document you view will be a time limit for appealing and where to fax or mail it. Be very detailed and factual.
