When submitting a sick leave letter to a college professor or administrator, there are a few things you need to make sure to include:

Here's a short sample letter you could submit to your college in order to get some sick leave approved:
To whoever it may concern,
I'm writing to inform you that I'm currently suffering from (name of the disease) and expect to be absent from my course (name of course) for (number of days).
I should be back by (date you expect to return), and will try my best to minmize the impact this sick leave will have on my progress on my course, but if there's any additional reading or preparation I can do from home, then please forward me details to (your email address).
