In my case it was simply the belt slipped a cog. I finally figured it out when I noticed the feed dawgs were trying to move the material when the needle was down.
To fix I removed the larger cover on the underneath of the machine, slid the belt off the gear, and matched the needle going down with the dawgs going down. Slid the belt back on and adjusted the belt tensioner a little more to hopefully alleviate it from happening again.
