Email the teacher, and schedule a meeting. Plan ahead for this meeting--do your hair, and wear a collared shirt under a sweater and chinos...they love that, whether you are a guy or a girl.
Then just be honest. Tell the teacher everything, and make sure you address WHY it is so important for you to take a leave. If that doesn't work, try the same thing with your principal or dean. : )
