'I recently applied for and was granted (enter amount of days) days of leave to, or for, (state reason for originally granted leave). Unfortunately, it has now become necessary for me to request an extension of this leave by (state number of required days) because (state specific reason why extended leave is required).
An additional paragraph could then be used to apologize for inconvenience, etc, and a promise to make up for the lost time on returning. This should then be followed by a request for the extended leave to be confirmed. Finally, the letter has to be signed.
Above all, it is important to be specific and honest about the reason for the request. If the request is granted and it turns out later that the reason was not a valid one, it could lead to disciplinary actions or, in some particularly severe cases, even to dismissal.
