I am a firm believer when it comes to things like this, that its best done in person as letter writing to your manager for something like this is kinda chicken and shows no respect here for anyone. Just face him and explain the situation and what you need to happen here. Chances are itll work too if done in person so have dates you need for a medical leave of absence and reason and just go do it
