Your bank must assigned a bank representative only for your companies work. With your company letter head paper and attention to your representative just type a single line requesting for y new cheque book.

Your company stamp and authorized signatory must sign down-left

Personal View:

It will change as per your country specific bank obligations. In general, you should approach to the nearest branch and they will let you fill a new cheque book request form.

At minor, only a letter requesting a new cheque book, signed by you will be enough. Bank will require your ID at all cases.
